On-going tenured faculty members and faculty members holding five-year ETA appointments are required to submit a dossier to their Academic Unit Head for Performance Evaluation on or before October 1 every two years (in even-numbered years only).
Please consult the current CUFA Collective Agreement, Article 14.14 for further information.

Faculty members in Arts and Science should download and use the Performance Evaluation Checklist (MS-Word format) and the appropriate Performance Review Worksheet - Performance Review Worksheet - ETA (MS-Word format) or Performance Review Worksheet - Tenured (MS-Word format) - when preparing and submitting their dossiers to their departments.
